What we’ll check
Height, weight, BMI, and blood pressure recorded on growth curve. Yearly vision screenings. Scoliosis screening via back examination. Evaluation of physical maturation changes. Oral health assessment. Screening for acne, piercings, and self-harm signs.
Immunizations due
Meningococcal conjugate vaccine #2 and Meningococcal B vaccine #1 at age 16. HPV #2 prior to age 16 (if not completed, HPV #3 given at this age). Meningococcal B vaccine #2 administered 6 months after first dose. Annual flu vaccine.
Doses follow the current AAP and CDC schedule — ask us anything about them at the visit.
Screenings
TB surveillance, anemia surveillance, dyslipidemia surveillance, adolescent depression screening, hemoglobin testing for females. PHQ-9 form provided at every well visit.
Safety at this age
Car safety during sports activities and water safety; sun protection; know teen's friends and families; maintain working smoke detectors and fire extinguishers with escape routes; avoid smoking around teen; ensure safe interactions with pets, trampolines, and swimming pools with proper equipment; remove or securely store firearms; keep Poison Control number (1-800-222-1222) accessible; discuss "safe sexual encounters" and risks; address domestic violence awareness and substance abuse risks.
Guidance for this age
School/development: Discuss "preparation for school or work beyond high school"; review school performance. Mental health: "We will provide a PHQ-9 form to complete during every well visit" to assess concerns; address stress management. Puberty/sexuality: Discuss sexual health including "pregnancy, sexually transmitted infections"; emphasize importance of "honest answers" regarding sexuality. Substance use: Address "alcohol, cigarette smoking, street drugs" with candid conversation during visits. Screen time: "Monitor computer/phone use daily." Driving: Complete driver's permit forms if age-appropriate.
Feeding
Offer variety of healthy foods including vegetables, fruits, and lean proteins; help teen choose nutritious options; encourage family meals and breakfast; limit sugar intake and encourage water consumption; engage in minimum 60 minutes daily physical activity; avoid fast foods, fried foods, and fatty foods; address body image awareness and diet concerns.
